The RER (commuter train) line A runs from Disneyland into the city. Change at Chatelet les Halles onto Line B and go one stop to Gare du Nord. Here is a map of the network:

The trains run quite frequently (every 20-30 minutes or so). Here is a schedule - it's a couple of years old but will give you an idea.

If you are unfamiliar with travelling by train in Paris, the homepage of this website, the one you're on now, Paris By Train, has detailed guides on the trains, the network, how to buy tickets etc. Go to:

    Regarding platforms, I would say:

At Disneyland, the station is small and it be will very easy to see what platform to go to. Simply look for RER Line A to Paris.

Chatelet les Halles is a much larger station. You need to follow signs to RER Line B in the direction Mitry-claye or Charles de Gaulle Airport (look at the map and you will see what I mean).

At Paris Nord, you will arrive in the RER (commuter) section of the station and will need to walk to the part of the station for long distance trains. Follow signs for TGV or Grandes Lignes. When you get to the mainline train area, there will be a large platform indicator showing you which platform your train departs from.
